Being vulnerable, sharing trauma, facing fears, and experiencing change can feel intimidating and isolating. However, I believe that with the right person by your side, growth and change are possible for everyone. I believe that each individual is created uniquely and is the expert of themselves and their life. That is why I enjoy getting to know my clients by meeting them where they are and building a strong foundation from there. This means we’ll tailor our sessions to your individual needs through collaborative efforts.
My job as your counselor is to follow you on your journey and teach you coping skills so you can have the tools to manage your daily stressors. I use a mix of therapy techniques and modalities to best serve each person and their unique needs. With varied life and work experiences, I have a personal familiarity with (but not limited to) foster care and adoption, diabetes, chronic illness, first responders (police, fire, EMS, medical), and the field of education.
My name is Elizabeth Crowley and I hold a Master of Science in Clinical Mental Health Counseling from Grand Canyon University. I’m a Licensed Associate Counselor (LAC), a Certified Clinical Trauma Professional (CCTP), and a Sandtray Therapist. At my private practice in Surprise, Arizona, I work with individuals – children ages 7+, adolescents, teens, adults, and seniors.
During my final year of studies, I completed an internship at The Meadows in Wickenburg, Arizona. I bring a strong set of skills working with clients impacted by mental health. My BA is in Human Services, and I have experience in treatment modalities including Solution Focused Therapy, Family Systems Therapy, Cognitive Behavioral Therapy, Dialectic Behavioral Therapy, Somatic Experiencing, Trauma Focused Therapies, and Sandtray Therapy.

I work with individuals – children ages 7+, adolescents, teens, adults, and seniors.
My therapy practice is private pay only. This means I do NOT accept insurance, but I can offer to prepare a “superbill” that you may submit to your insurance if they happen to cover out-of-network therapy visits with a supervised practice. You would need to contact them directly to confirm your coverage and be sure to mention “supervised practice”.
My rates are $200 for the initial session and $180 for each 50-minute session thereafter. If there is a crisis, the rate is $250 for the initial 90 minutes. You are responsible for the full cost of the session if it is canceled within 48 hours.

I offer telehealth therapy on a case-by-case basis, however, I prefer working with clients in-person in my office in Surprise. In-person visits allow for a more authentic connection and gives us the opportunity to utilize Sandtray therapy should we want to take advantage of that experiential technique.
Open Minds Counseling, PLLC is located in the Phoenix Northwest Valley. It is at 15027 W Bell Road, Suite 150-A6 in Surprise, Arizona – near the 303. This location is convenient for individuals living or working in Surprise, Wickenburg, Whittman, Waddell, Peoria, Glendale, & Goodyear.
It means that I have graduated with my master’s degree in counseling and I am licensed by the Arizona Board of Behavioral Health as a LAC (Licensed Associate Counselor), and then every LAC has to put in 3,200 hours of supervised work before becoming independently licensed as an LPC (Licensed Professional Counselor). Therefore, my practice is supervised by an LPC who is independently licensed and has access to all client notes and information.
YOUR RIGHTS AND PROTECTIONS AGAINST “SURPRISE” MEDICAL BILLS (OMB Control Number: 0938-1401)
When you get emergency care or get treated by an out-of-network provider at an in-network hospital or ambulatory surgical center, you are protected from surprise billing or balance billing.
What is “balance billing” (sometimes called “surprise billing”)?
When you see a doctor or other health care provider, you may owe certain out-of-pocket costs, such as a copayment, coinsurance, and/or a deductible. You may have other costs or have to pay the entire bill if you see a provider or visit a health care facility that isn’t in your health plan’s network.
“Out-of-network” describes providers and facilities that haven’t signed a contract with your health plan. Out-of-network providers may be permitted to bill you for the difference between what your plan agreed to pay, and the full amount charged for a service. This is called “balance billing.” This amount is likely more than in-network costs for the same service and might not count toward your annual out-of-pocket limit.
“Surprise billing” is an unexpected balance bill. This can happen when you can’t control who is involved in your care – like when you have an emergency or when you schedule a visit at an in-network facility but are unexpectedly treated by an out-of-network provider.
According to Arizona’s Department of Insurance & Financial Institutions website:
Balance billing – or a surprise medical bill – happens when you get a bill from a doctor, laboratory, durable medical equipment provider, or other health care provider who isn’t part of your health plan’s network. Often, consumers didn’t know they were getting care from out-of-network providers. For example, a patient goes to an in-network hospital for emergency care and is treated by an out-of-network doctor. The doctor and the hospital each bill $1,000 for their services, and the health plan pays them each $400. The in-network hospital can only bill the patient for copays, deductibles, and coinsurance amounts. The doctor, however, may bill for the copays, deductibles, and coinsurance as well as any other amounts that the health plan did not pay.
You are protected from balance billing for:
Emergency services
If you have an emergency medical condition and get emergency services from an out-of-network provider or facility, the most the provider or facility may bill you is your plan’s in-network cost-sharing amount (such as copayments and coinsurance). You can’t be balance billed for these emergency services. This includes services you may get after you’re in stable condition unless you give written consent and give up your protections not to be balanced billed for these post-stabilization services.
Certain services at an in-network hospital or ambulatory surgical center
When you get services from an in-network hospital or ambulatory surgical center, certain providers there may be out-of-network. In these cases, the most those providers may bill you is your plan’s in-network cost-sharing amount. This applies to emergency medicine, anesthesia, pathology, radiology, laboratory, neonatology, assistant surgeon, hospitalist, or intensivist services. These providers can’t balance bill you and may not ask you to give up your protections not to be balance billed.
If you get other services at these in-network facilities, out-of-network providers can’t balance bill you unless you give written consent and give up your protections.
You’re never required to give up your protection from balance billing.
You also aren’t required to get care out-of-network.
You can choose a provider or facility in your plan’s network.
